Firstly, please check with the receptionists that you live in our catchment area; this is approximately a 10 mile radius of the surgery. They will then give each new patient two forms to fill in.

One form is sent off to the Health Authority requesting your full UK medical history from your previous surgery. The other form allows us to obtain a brief summary of this information while we wait for your full records.

As soon as these forms are filled in and the data entered on to our computer, you are able to see any of the medical staff in the practice. Please Note : Appointments can be made prior to you filling in these forms, but we need to have you registered onto our computer system before you see a member of our medical staff.
