From October 2007 the practice is pleased to offer a prescriptions home delivery service.

Do you or someone you know experience difficulty in collecting repeat prescriptions?

Are you aged 60 or over?

Are you exempt from prescription charges or hold a medication pre-payment certificate?

If you can answer yes to any of the questions above and would like to have your prescriptions delivered, please complete the consent form (which you can download by clicking on the link below) and forward onto the practice. A member of the Dispensing Team will be in touch.

If you have given permission on the consent form, the delivery driver can leave your medication at your premises if you are not at home. Many items are too large to be posted through the letter box so it is useful to clearly identify a suitably secure place to leave your medication.

You can identify repeat items on the repeat slip attached to your medication and return the slip to the delivery driver who will arrange for delivery at the next appropriate date.
