Travel Clinic.
If you are travelling abroad, you
may require extra immunisations and
injections. In order that we can
provide you with these, we need to
know the full details of your travel
plans. Please book a double
appointment to come in and discuss
your requirements with a Practice
Nurse and where appropriate have the
relevant vaccinations for your trip.
Prior to your appointment please
complete the Travel Form and bring
it to the appointment with you.
A copy of the
travel form can be downloaded by
clicking here and is
also available from Reception. It is
imperative that you complete all of
the questions including name as it
is written on your passport and
passport number. In the case of
yellow fever vaccinations your
certificate cannot be issued without
this information.
Please note that we need advance
notice of your travel plans if you
telephone less than 4 weeks before
your planned travel date, we may
have to redirect you to another
travel service.
A fee is payable for some
vaccinations required for travel
purposes. Details of these charges
are available at the surgery and
also on the web site under Patient
services, Non NHS fees.
